About the Author
A pediatrician with forty years of experience, Marc Weissbluth, M.D., is also a leading researcher on sleep and children. He founded the original Sleep Disorders Center at Chicago’s Children’s Memorial Hospital (now called the Ann and Robert H. Sleeplessness in children and worrying about sleeplessness have been around for a long time.

Healthy sleep appears to come so easily and naturally to newborn babies. Effortlessly, they fall asleep and stay asleep. Their sleep patterns, however, shift and evolve as the brain matures during the first few weeks and months. Such changes may result in “day/night confusion”—long sleep periods during the day and long wakeful periods at night. This is bothersome, but it is only a problem of timing. The young infant still does not have any difficulty falling asleep or staying asleep. After several weeks of age, though, parents can shape natural sleep rhythms and patterns into sleep habits.

It comes as a surprise to many parents that healthy sleep habits do not develop automatically. In fact, parents can and do help or hinder the development of healthy sleep habits. Of course, children will spontaneously fall asleep when totally exhausted—“crashing” is a biological necessity! But this is unhealthy, because extreme fatigue (often identified by “wired” behavior immediately preceding the crash) interferes with normal social interactions and even learning. You should not assume that it is “natural” for all children to get peevish, irritable, or cranky at the end of the day. Well-rested children do not behave this way.

Before electricity, radio, television, computers, or commuting long distances to work, children went to sleep earlier than children do today. Our current popular late bedtimes may be no more “natural” than the outdated “natural” belief that fatter babies are healthier babies. Commonly held or popular beliefs about what is natural, normal, or healthy are not always true. In addition, when you think of child rearing, it may appear “natural” for you to consider parenting practices performed in traditional cultures. That is, breast-feed frequently day and night and sleep with your baby, wear your baby in a sling or soft carrier, always be close to your baby, and always respond to your baby. This is not always practical for some families, and even for those families who choose this “natural” style, their baby’s extreme fussiness/crying/not sleeping or “unnatural” factors can interfere.

Dr. Christian Guilleminault, who along with Dr. William C. Dement was the founding editor of the world’s leading journal of sleep research, taught me to consider five fundamental principles of understanding sleep:

1.      The sleeping brain is not a resting brain.

2.      The sleeping brain functions in a different manner than the waking brain.

3.      The activity and work of the sleeping brain are purposeful.

4.      The process of falling asleep is learned.

5.      Providing the growing brain with sufficient sleep is necessary for developing the ability to concentrate and an easier temperament.

Sleep is the power source that keeps your mind alert and calm. Every night and at every nap, sleep recharges the brain’s battery. Sleeping well increases brainpower just as lifting weights builds stronger muscles, because sleeping well increases your attention span and allows you to be physically relaxed and mentally alert at the same time. “Natural”

All babies have spells of fussing and crying.

Relatives and friends want to help soothe the baby and are expected to assist parents.

Breast-feeding and sleeping with your baby are powerful ways to soothe your baby.

Urban stimulation (noises, voices, delivery trucks, shopping trips, errands) may interfere with baby’s sleeping.

Day care (not being able to put your child to sleep when just starting to become tired or too much stimulation) may interfere with baby’s sleeping.

Social isolation forcing only the mother to be wholly responsible to take care of soothing and sleeping may cause intense stress for the mother.

Busy modern lifestyles means that parents have many things to do and little time to do them; sometimes they have to take their baby with them even at sleep times.

Mothers have to work outside the house, miss playing with their baby, and keep their baby up too late at night.

Fathers or mothers have a long commute and return home from work late, want to play with their baby, and keep their baby up too late at night.

Grandparents interfere with sleep routines.

The notion of optimal wakefulness, also called optimal alertness, is important, because we tend to think simplistically of being either awake or asleep. Just as our twenty-four-hour cycle consists of more than just the two states called daytime and nighttime, there are gradations—which we call dawn and dusk—in sleep and wakefulness.

In sleep, the levels vary from deep sleep to partial arousals; in wakefulness, the levels vary from being wide awake to being groggy.

The importance of optimal wakefulness cannot be overemphasized. If your child does not get all the sleep he needs, he may seem either drowsy or hyperalert. If either state lasts for a long time, the results are the same: a child with a difficult mood and hard-to-control behavior, certainly not one who is ready and able to enjoy himself or get the most out of the myriad of learning experiences placed before him.

With our busy lifestyles, how can we keep track of nap schedules and regular bedtime hours? Is it really true that I can harm my baby by giving him love at night when he cries out for me? How can I be sure that sleep is really that important? Am I a bad parent if my child cries? If he cries at night, isn’t he feeling insecure? These are questions many parents ask me. Parents will often mention that articles or books they have read seem to support different ideas, and so they conclude by saying that since this whole issue is “so controversial,” they would rather let matters stay as they are. If you think your child is not sleeping well and if you disagree with the suggestions in this book, then ask yourself how long you should wait for improvement to occur. Three months? Three years? If you are following the opinion of a professional who says you must spend more time with your child at night to make him feel more “secure,” ask that professional, “When will I know we are on the right track?” Don’t wait forever. Consider what Dr. Charles E. Sundell, the physician in charge of the Children’s Department in the Prince of Wales General Hospital in England, wrote in 1922: “Success in the treatment of sleeplessness in infants is a good standard by which to estimate the patience and skill of the practitioner.” He also wrote: “A sleepless baby is a reproach to his guardian, and convicts them of some failure in their guardianship.” So don’t think that worrying about sleeplessness is just a contemporary issue.

The truth is, modern research regarding sleep/wake states only confirms what careful practitioners such as Dr. Sundell observed over eighty years ago. He wrote:

The temptation to postpone the time for a baby’s sleep, so that he may be admired by some relative or friend who is late in arriving, or so that his nurse may finish some work on which she may be engaged, must be strongly resisted. A sleepy child who is kept awake exhausts his nervous energy very quickly in peevish restlessness, and when preparations are at last made for his sleep he may be too weary to settle down. . . .

Regularity of habits is one of the sheet-anchors by which the baroque of an infant’s health is secured. The reestablishment of a regular routine, after even a short break, frequently calls for patient perseverance on the part of the nurse, but though the child may protest vigorously for several nights, absolute firmness seldom fails to procure the desired result.

Each baby is unique. They’re like little snowflakes. Babies are born with individual traits that affect the amount of physical activity, the duration of sleep, and the length of periods of crying they will sustain. But babies also differ in more subtle ways. Some are easier to “read”; they seem to have predictable schedules for feeding and sleeping. These babies also tend to cry less and sleep more. Regular babies are more self-soothing; they fall asleep easier, and when they awaken at night they are more able to return to sleep unassisted. But don’t blame yourself if you have an irregular baby who cries a lot and is less self-soothing. It’s only luck, although social customs may affect how you feel about it.

In those societies where the mother holds the baby close all the time, and her breasts are always available for nursing and soothing, there are still great differences among babies in terms of fussiness and crying. The mother compensates by increasing the amount of rhythmic, rocking motions or nursing. She may not even expect the baby to sleep alone, away from her body. As she grows up, a child might share the bed with her parents for a long time. This is not necessarily good or bad; it’s just different from the expectations of most middle-class Western families.

So not only do babies sleep differently, but every society’s expectations condition parents’ feelings in different ways. Remember, there are no universally “right” or “wrong” ways, or “natural” versus “unnatural” styles, of raising children. Less-developed societies are not necessarily more “natural” and thus “healthier” in their child-rearing practices. After all, strychnine and cow’s milk are equally “natural,” but they have altogether different effects when ingested.

How much we are bothered by infant crying or poor sleep habits might partially reflect our own expectations about how to be “good” parents. Do we want to carry the baby all the time, twenty-four hours a day, or do we want to put the baby down sometimes to sleep?

Here’s a true story. A Saudi Arabian princess came to my office for a consultation, accompanied by her English-trained Saudi pediatrician, her English-trained Saudi nanny, and two other women, to discuss sleeping habits for the royal family’s children. The pediatrician described child care arrangements that had been popular among British aristocrats in the nineteenth century. Like trained baby nurses in nineteenth-century England, the Saudi Arabian nanny was always able to hold the princess’s baby while the child was sleeping for the simple reason that the Saudi nurse had her own servants! These subordinate nannies were not as well trained and were assigned the menial domestic chores associated with child rearing.

The majority of parents do not have child care staffs. They have to rely on their own skills. So if we are greatly bothered by our baby’s crying or our guilt about not being “good” parents, this may interfere with our developing a sense of competence. We may feel that we cannot influence sleep patterns in our child. Unfortunately, this way of thinking can set the stage for future sleep disorders.

Sleep problems not only disrupt a child’s nights, they disrupt his days, too, by making him less mentally alert, more inattentive, unable to concentrate, and easily distracted. They also make him more physically impulsive, hyperactive, or lazy. But when children sleep well, they are optimally awake and alert, able to learn and grow up with charm and humor. When parents are too irregular, inconsistent, or oversolicitous, or when there are unresolved problems between the parents, the resulting sleep problems converge, producing excessive nighttime wakefulness and crying.

Please do not simply assume that children must pass through different “stages” at different ages, and that these stages inevitably create sleep problems. The truth is that after three or four months of age, all children can begin to learn to sleep well. The learning process will occur as naturally as learning how to walk.

The bad news is that some parents create sleep problems. The good news is that parents can prevent sleep problems as well as correct any that develop.

Parents who favor a more gradual approach (controlled crying or graduated extinction) over an abrupt approach (ignoring or extinction) often complain of frequent “relapses.” The general reason why a gradual approach tends to be less successful in the long run is that it takes longer and there are always natural disruptions of sleep, such as illnesses or vacations. The subsequent reestablishment of healthy sleep routines using a gradual approach becomes very stressful to the parents. Several days or weeks of a gradual approach often wear down parents, so they give up and revert to their old inconsistencies. Parents who have successfully used extinction know that they might have one, and only one, night of crying after they return home from several days on vacation or from a visit to a relative’s house.

The truth is that some parents swing back and forth between firmness and permissiveness so often, they cannot make any cure stick. They often confuse their wishful thinking with the child’s actual behavior. This is why a sleep log, which I will describe later, can be an important tool to help you document what you are really doing and how your child is really responding. After all, short-term “successes” might only reflect brief periods when your child crashes at night from chronic exhaustion. Or the actual improvement in sleep habits may be so marginal that the normal disruptions of vacations, trips, illnesses, or other irregularities constantly buffet the still-tired child and cause repeated “relapses” in which he wakes often during the night or fights going to sleep.

In contrast, parents who successfully carry out an abrupt retraining program—the cold-turkey approach—to improve sleep habits see immediate and dramatic improvement without any lasting ill effects. These children have fewer relapses and recover faster and more completely from natural disruptions of sleep routines. Seeing a cure really “stick” for a while gives you the courage to keep tighter control over sleep patterns and to repeat the process again if needed.

I cannot emphasize enough how important it is for parents to start early to help their child learn to sleep well.

PRACTICAL POINT

If you start early with sleep training, you will be well along the path to preventing sleep problems.

When you start early, there are no long bouts of crying and no problems with sleeping. The process of falling asleep unassisted is a skill, and as with any other skill, it is easier to teach good habits first than it is to correct bad habits later. Also, as with any other skill, success comes only after a period of practice.

The many personal accounts in this book, contributed by a variety of caring, thoughtful parents, should add extra incentive to teach healthy sleep habits early or to make a change to correct your child’s sleep problems right now, so that you can all get on with the best part of having children—enjoying them! Some parents may need professional help to establish reasonable, orderly home routines, to iron out conflicts between parents, or to help an older child with a well-established sleep problem. To maintain healthy sleep for your young child, you need the courage to be firm without feeling guilt or fear that she will resent you or love you less. In fact, the best prescription I can offer is to create a loving home with a well-rested child and well-rested parents.

Only suggestion would be a FAQ section

I am a first time mom of a baby who just turned 4 months old a couple of days ago. I was given this book when she was born, but never read it because she was an amazing sleeper. She slept on her own almost anywhere, through anything. At about 1 1/2 months she started having a "fussy time" at around 8 or 9pm that lasted until about 10:45 when she would fall asleep. It wasn't crying, it was just fussy. Otherwise, she was still a great sleeper (and still sleeping through the night since a week home from the hospital). At almost exactly 3 months old, however, she started screaming and was hysterical at her normal bedtime. The first time, she was inconsolable. She screamed for 4 hours. Nothing I did worked, until I climbed into the bath tub with both of us fully clothed and let her float with me in the water. She calmed down until we took her out. But at that point, she would then take her bottle and soon fell asleep. From that point on, getting her to sleep at night was a crying session every night for her (maybe about 1/2 hour), and she no longer would nap unless it was ON ME in the day time. When I told the doctor, they suggested I read this book. I already had it! So I went home that day and read the entire thing (aside for the stuff for school aged and teenagers). I did NOTHING all day but watch my baby for cues. I had NO IDEA, nor had ANYONE told me that babies shouldn't stay up longer than 2 hours. My baby was up for sometimes 4 hours! Just because she COULD do it didn't mean she SHOULD! That's one of the biggest points I took from the book. I learned that at 3-4 months she was a different baby now, and so things had to change, and that I was the one that had to start to implement that change. After watching her From the time she woke up from a nap, I realized her eyelids started getting pink at about 45 min after waking up, and at about 1 hour, she would turn her head slowly to the left (people had previously told me this was called "rooting" and that she was hungry, so I would always try to feed her when I saw this, but she always protested... Because that's not what it was! She does this EXACT same thing no matter where she is. If she's laying on her back on the floor, if she is in her bouncy seat, or if I am holding her. And it is 90% of the time turning to her left side. That is her CUE! If I whisk her upstairs at that moment, I can sing her 1 verse of twinkle twinkle little star as I cuddle her, then set her into her bed, hand her a snuggly, she does a little whimper, then falls asleep. If I MISS the cue, she will start the rub her eyes and fuss. If I don't get her in her bed soon enough, she will cry when I put her down, and will require the bottle to soothe her, and will not fall right asleep.
After learning her cue THAT FIRST DAY, I put her right in her dark room (black out curtains) with a white noise machine ($9.99 travel one from buy buy baby) and she sleeps for 1 1/2 hours. She STIRS at 45 minutes EXACTLY every single nap, but if I leave her alone, she will go back to sleep for another 45 minutes, and then wakes up at EXACTLY 1 1/2 hours from when she fell asleep. It will be only ONE WEEK TOMORROW that I have been doing this, PLUS we went away on vacation two days ago, and we are still following the plan, and she is DOING IT. She went to sleep at 8:30 last night, slept until 7am, took a bottle, had a diaper change, played for a short time, then fell asleep at exactly 8am and slept again until 9:30. This book has not only changed my life as a first time mom, but I also teach 1st grade, and it has changed the way I look at ALL KIDS... SOOOOOO many issues I see in school every day I now know are most likely caused by these children having either too little sleep, too erratic sleep, or fragmented sleep. The only complaint I have is that there is not a FAQ section in the book. I think it would be a HIGHLY valued section. The random questions throughout the book would be better served in a FAQ section that can be quickly referenced and re-referenced, and more questions/answers should be added.

I also wish that Dr. Weissbluth had a website or email address that you could ask questions, because even though my baby is now sleeping on naps and "through the night" again, I have no idea why she is only awake for 1 hour at a time during the day. The standard suggestions of amount of naps and wake times don't apply to her, and I want to know when/if that will change. She is 4 months old, and her "morning nap" after a wake-up time of 7am comes at exactly 8am (she cues me at 7:45 and she's sound asleep by 8), which is NOWHERE NEAR the suggested 9am nap. Then, throughout the rest of the day, she can ONLY stay up for 1hr - 1 hour and 15 min. 1 1/2 is REALLY pushing my luck, and it has NEVER been 2. She takes FOUR naps per day, with a 7:30 (before the time change yesterday, so I don't know how that will screw things up) bed time, and a couple of times, she has needed FIVE naps. Shouldn't she be able to stay up longer than 1 hour at 4 months? That's pretty much my only concern.

Very insightful

This was recommended to me by several people. It is very long and wordy, but the introduction advises you to skip to the parts you need, when you need them. I learned I was already doing most of the things Dr. Weissbluth suggested. My baby sleeps well, but nurses often. This book can't solve that problem! However, the first time I changed my son's nap time according to the book, he slept 2 hours (previously never more than 45). I can see how this book will be helpful to people who want to teach healthy sleep habits. I recommend reading this before your baby is 3 months so you know what to do before he/she starts developing sleep habits.

I love how informational this book is! It really opened my mind to how important sleep is for my little one. 50 Lush Color Photos Are Worth More Than 50,000 Words

Although this book has many elements, its core is the Harley-Davidson as a design. The photographs are mostly of 50 different models, focused on the motorcycle alone, done with remarkable lighting to create exquisite detail and color. You will feel like you are visiting a museum of mint Harleys as you "stroll" from page to page while sitting comfortably in your favorite chair. The book is strengthened by some classic Harley photos that you have seen before, such as from Easy Rider. There is also a good overview of the development of the company, key technologies, and a timeline of when different bikes were introduced. For those who don't know about Harleys, you also have a glossary to help you interpret, and a list of contacts to help you get even more information.
"People remember their first Harley-Davidson." As Hugo Wilson points out, that's because "Harleys look different, sound different and certainly ride different to other motorcycles." One of the strengths of the book is that it provides many helpful details about the bases of those differences. I could literally "hear" a Harley every time I looked at one of the images. And now I know why they sound that way. You will, too, when you read this exciting book.
"Their extraordinary appeal makes small boys dream and grown men save." Harley-Davidsons clearly draw on our sense of adventure, our desire for freedom, and our need to be different. I am not an expert on motorcycles but have spent many happy hours on them. I could feel the thundering power in my legs as I looked at the Harley in Terminator II, the 74FLHB Electra Glide in blue from 1965, the FLHS Electra Glide from 1988, the FLSTF Fat Boy from 1999, and especially the Buell X1 Lightning from 1999.
To me the best page was 184 where there is a cutaway of the famous V-twin engine, along with a good description of each element. This is the heart of what makes a Harley a Harley, and I was glad to learn more about this wonderful design.
After you have finished enjoying this book and taken a ride on your Harley to celebrate (don't have one yet? . . . well, start dreaming and saving . . .), I suggest that you think about other ways you can enjoy your spirit of adventure and desire for the open road. Riding a Harley doesn't get you all the exercise you could use. Are there any hiking trails that you could ride your Harley to? In that way, you could have a double adventure. Plus, you can have even more enjoyment with your family and friends while you hike. If you haven't hiked the Grand Canyon yet down to the Colorado River, that would make a great ride/hike combination. Just do it! -- (but do plan ahead, you'll enjoy it more if you do).

A quality look at the history of Harley

Once you hear that iconic rumbling, you know there is a Harley-Davidson around. Harley-Davidsons are everywhere, and there is a solid following of motorcycle enthusiasts all across the world. The Ultimate Harley-Davidson book breaks down each model decade by decade. It is a wonderful format for Harley-Davidson lovers who enjoy reading the history that goes along with these wonderful machines. The entire book is bright and bold with complete and accurate information and outstanding photographs from past to present.

From the early days of the company to the days of Easy Ride, the reader gets a deep look into Harley-Davidson culture, specs, and mechanics. "The arrival of high-spec Japanese imports had another consequence. People wondered why, if Japanese 125cc machine could have an electric starter, and American 1200cc machine could not. Harley's response was to fit an electric starter to its FL models from 1965 to create the Electra Glide, possibly the most famous motorcycle model ever built. It even had a film named after it, Electra Glide in Blue (1973)."

Hugo Wilson, author and motorcycle enthusiast, has written many best-selling books on motorcycles and is the editor of the United Kingdom's top selling motorcycle magazine "Bikes". I think of him as a British Allan Girdler. Girdler is an American author and motorcycle enthusiast who has educated me well on the history of the Harley-Davidson Xr-750 and Harley Davidson Sportster.

Ultimate Harley-Davidson is a great collectors item or gift! This book is well planned, and the decade by decade breakdown will help Harley lovers explore each era. The index is also bright and vivid for easy searching.

I read this book about 20 years ago and I am enjoying the pleasure of reading it again. The characters in the book are real for me. It's well written and the story flows from decade to decade.

An engaging read

A fine novel, now somewhat dated after a quarter century, about Declan Walsh's journey from soldier to Supreme Court chief justice to monk to Pope. Told as narrations, being told to a reporter who is a minor character in the novel. The Korea sequence and the Papal portion (especially the retelling of the Papal election) are the best parts--time has passed by the court portion. The epilogue is very good as well.

A vivid portrayal of an imaginary "great man."

This is an extraordinary novel of the same genre as "The Shoes of the Fisherman." It portrays the imaginary life of one Declan Walsh. The author's stylistic method is extraordinarily effective. The author uses as a setting a reporter interviewing three different (imaginary) intimates of Walsh. Each of these interviewees: a Marine Corps gunnery sergeant, a Supreme Court Justice, and a Catholic Cardinal, paints a different portrayal of Walsh, and incidentally also presents the reader with a fascinating character study in his own right.
In the novel, Declan Walsh lives an extraordinary life: he is a Medal of Honor winner in Korea, he later becomes Chief Justice of the United States. He resigns his post on the Supreme Court in the face of personal tragedy, and later is selected as Pope by a deadlocked Church conclave.
My favorite portion of the novel is the first third, which deals with Walsh as a Marine Corps battalion commander in Korea. The depiction of men in combat, the leadership issues, the life and death decisions, and above all the hard fighting and hard dying are powerfully done. The reader can feel the cold Korean night, and share the Marines' honest fear and bravery as tough infantry combat rages. I have never read a more realistic and authentic story of men in combat.
The second section of the novel deals with Walsh as Chief Justice. The legal issues are pretty much passe' now--they deal with issues circa 1964-1975. However, this portion of the novel retains its relevance despite this because it gives an interesting insight into the decision making process within the Court, and again the portrayal of Walsh is wonderfully done.
The last section of the novel deals with Walsh as Pope. It is not dated in any significant way, because the Church has not grappled with most of the issues portrayed in the novel. Again, the author takes us into the Catholic Church and explains some of the politics, infighting, and differences in ideology that permeat the world's oldest beauracracy. One need not be a Catholic to find this fascinating.
The best thing about this novel is the quality of the writing. Murphy's prose is clear, wonderfully stylistic, and extremely entertaining. Many of the characters in the novel literally seem to spring to life, so well done are Murphy's characterizations. Murphy's characters act and seem like real people living real lives at all times. I have read this novel many times over the years and I regard it as one of the best reads I have ever experienced. Highly recommended.

Five Days You Won't Forget

The first half of this book reads like an apocalyptic thriller while the second half is like a legal drama and in fact was dramatized on the television show, Boston Legal.
The tragedy that occurred at Memorial Hospital in the aftermath of Hurricane Katrina is unflinchingly detailed by the author. The horrors that the staff and patients had to face will haunt you. The actions that were taken to save lives was heroic. There were also decisions made, however, that led to at least 7 deaths. Were these unavoidable casualties of the disaster or were these people murdered to effect a long overdue rescue of the remaining patients and staff?
The questions surrounding the deaths led to an investigation of one doctor and a couple of nurses. This legal investigation is what comprises the second half of the book.
The author, a Pulitzer Prize winning journalist, gives a fair and complete assessment of the five day ordeal in the storm ravaged hospital. There is also background information for many of the staff and some patients who were stranded in the flood. There were points that actually had me near tears as I read of their struggles.
The investigation after the incident and the legal battles which ensued are equally as compelling. The political machine that took over so much of the Katrina recovery is a big part of the story. The reader is left to come to his own conclusion based on the information given. Through the stellar reporting of this author, it is easy to empathize with both sides in some respects. Was euthanasia necessary? I'm not going to go into my own personal beliefs but I will say that you will look at the whole situation differently after reading this book.
You will also be forced to take a look at the choices available to us as the end of life approaches.
Five Days at Memorial is a compelling book that documents a particularly horrible natural disaster and the mess that our government and some corporations made of the rescue process. You can't read this and not be changed. It isn't a story that has a happy ending but it's the kind of thing that needs a light shown on it so that, hopefully, we learn a valuable lesson from the mistakes that were made.

For those prepared to grapple with major ethical issues

This is not the type of book that anyone should pick up if they are not prepared to deal with some extremely grim realities. Although certainly all of us have heard, and some of us have experienced personally, the horrors resulting from "natural disasters", Sheri Fink's exhaustively detailed description of events at Memorial Hospital in New Orleans after Hurricane Katrina is uniquely powerful.

This is a book that SHOULD make one angry as well as profoundly and deeply distressed. It is also a fascinating investigation of the evolution of human perspective as "technical medicine" has become ever more complex and infrastructure-dependent. It includes an extraordinarily focused discussion of the historical issues - the fact that this hospital was flooded previously, but the necessary upgrades to provide better protection for the backup generators as recommended was never accomplished, being the most cogent.

For anyone who is not aware of the background story: As well as having a complement of "regular" patients, some in Intensive Care after such things as open heart and cancer surgery, the hospital had an organization called LifeCare that leased space on the 7th floor of the main hospital complex. Their patients were mostly elderly, and were in long-term care for extremely debilitating conditions that required extensive life support and monitoring, including dialysis, tube feeding, ongoing oxygen therapy, and so on. When the hurricane first approached, on Saturday, August 27, 2005, it seemed wise to move LifeCare patients in from a less secure, smaller facility in St. Bernard Parish, to the much larger location at Memorial. In addition many others, including family members of hospital staff and patients, along with their pets, and some other community members, sought refuge at the hospital, which was deemed to be a safe location. Therefore when the hurricane actually struck on Sunday, August 28th, there were many more people in residence than the normal load.

There was considerable damage from the storm itself, but everything seemed to be on target for recovery until the flooding of Monday, as of course was the story for the entirety of New Orleans. Sadly, at this point, the failure to upgrade the generators became a very serious issue, and for the following four days, conditions went from difficult to impossible to horrific. It became obvious as power failed and violence and looting became ever more prevalent in the surrounding neighborhoods that rescue and evacuation was not going to be practical, especially for the most severely debilitated patients. Although the hospital DID have a helipad, which turned out to be marginally accessible, for some reason it never seemed possible to launch a major evacuation by air. Eventually, on the fourth day (Thursday), when rescue efforts seemed completely stalled and many of the "critical" patients were rapidly deteriorating due to the extreme heat, lack of support systems for their therapies, and so on, the decision was made that anyone who had a DNR order in place and was not easily transportable would be given an overdose of morphine. The second half of the book deals with the legal repercussions faced by the medical professionals who made these fatal decisions.

Obviously, in past eras when the "promise" of high tech medicine was not so deeply ingrained in the minds of both the medical community and the general public, these issues would not have arisen. People who were terminally ill would either not even BE in a hospital setting, or if they were, it would be clear that removing them would be inevitable, whether they died in the process or not. One of the poignant scenes in the narrative is the story of the euthanizing of some of the pets who had been brought to the hospital for shelter "because there was no room for them in the rescue vehicles". At that point, it struck me as symbolic that perfectly healthy animals, considered "family" by many of their humans, were summarily executed for lack of "space". Meanwhile, there was all the agonizing about the failure to find room in those same vehicles for admittedly terminal humans with their wheelchairs and other "paraphernalia", and the subsequent choice of the medical professionals to ease (while perhaps speeding up) the passing of those humans.

I would recommend Fink's book only to those who are prepared to be courageous in thinking about these torturous life-and-death issues, and who really want to grapple with some of the major ethical concerns of our era.

Fink takes on a story with moral and ethical overtones- what killed 45 patients at Memorial Hospital in New Orleans in the days following Hurricane Katrina, in her incredibly fascinating Five Days at Memorial. Fink interviewed dozens of people who were there- doctors, nurses, aides, family members, patients, hospital administrators, rescuers, police investigators, coroners and more to tell her gripping story.

Fink drops the reader right into the hospital during the hurricane and in the horrific aftermath of the storm, when the levees failed and the hospital was completely surrounded by floodwaters. The reader feels the rising panic as generators fail, toilets stop working, medicines run low, cell phones die and communication is lost with the outside world.

Close to 200 people were evacuated from the hospital by helicopter and boats, but 45 patients died, most of them either terminally or gravely ill, the most of any hospital in the city. And most of them died of an overdose of morphine and Versed, allegedly by the hands of Dr. Anna Poe, a surgeon at the hospital. She and two nurses were arrested for killing those patients after a lengthy investigation.

Fink methodically lays out what went on at the hospital during those days. The corporate owner of the hospital, TenetCare, had an emergency plan that lacked some key elements. After 9/11, hospitals had to beef up emergency plans.
"The doctors at Memorial had drilled for disasters, but for scenarios like a Sarin gas attack, modeled that April, where multiple patients arrived at the hospital at once. Not in all his years of practice had Thiele drilled for the loss of backup power, running water, and transportation."
They had no contract with helicopter companies to evacuate the hospital (as other hospitals did) during a flood in a city where hurricanes and floods can be devastating. The person left in charge in the home office of Houston had no disaster experience or training, and the lack of communication with the hospital during the crisis was unconscionable.

The staff at Memorial felt they had been abandoned by their owners, as well as by their government. There seemed to be no one in charge at a local, state or federal level who could give them information as to when and how they would evacuate. All they heard were rumors of rampant looting, and the gunfire they could hear in the neighborhood made them fear they would be overrun by criminals looking for drugs.

When the evacuations begin, with boats commandeered by an older couple looking for a family member, and helicopters fly in, the triage that took place was the opposite of most; instead of the sickest going first, the healthiest patients were evacuated first. That decision had repercussions that had to be answered for later.

The first half of the book will have you on the edge of your seat, and the people who stayed behind to help the patients were heroic in their efforts. Fink sketches them with deserved empathy and compassion. As you read, you may ask yourself, "could I have done that?"

The second half of the book deals with the efforts by investigators and the state attorney general, who was looking to make a name for himself, to bring murder charges against the three women. Most of the general public did not feel this was warranted, and support for the women was strong.

But there were people, including doctors from Memorial, who were appalled at what happened and wanted to see justice for the people who died. They felt that these women violated their oath to do no harm and took matters into their own hands.

The one overriding theme of Five Days at Memorial is that governments and healthcare facilities must have effective disaster planning. There were so many failures on the part of government and corporations that allowed this to happen when it did not have to happen.

You may think you know how you feel about this situation, but Fink skillfully shows you all sides, and you will most likely come away from this book with more questions than answers as I did. This is a must-read book.

About the Author
Jessica James joined the Citibank FX Risk Advisory Group from Bank One, where she headed their Risk Advisory and Currency Overlay group in Europe. Her group was responsible for the design, marketing and maintenance of currency overlay strategies, and the provision of bespoke research to Bank clients, supporting internal marketing. At Citi, she is closely involved in both these areas, working with marketers and clients to uptier relationships. Jessica is well known for her research, with several books to her credit and a regular publication record in the financial press. Additionally, she teaches a number of financial mathematics courses and sits on the board of the Journal of Quantitative Finance, and the ICBI finance conference board. She has participated in several government Task Forces and is involved with the Institute of Physics as a member of their governing body and a member of their Industry and Business Board. The major part of Jessica's current research is in the FX area. Over the last few years she has been at the forefront of development of currency risk management models and overlay strategies, and has pioneered the use of Extreme Value Theory to manage the risk of large FX moves. Additionally, she has a wealth of experience in the practical process of setting up currency overlay as a business unit, including trading strategy testing, design and marketing. At Bank One, she was in charge of trading sheet design and strategy execution, and instigated and specified a continuous testing process to ensure that the strategies performed within the bounds of expectations. In addition to her FX research, Jessica is known for her work on interest rates, having written an extensive work on modelling and valuation (Interest Rate Modelling, Wiley 2000). She has also published in the credit and risk management areas. Prior to her career in finance, Jessica lectured in physics at Trinity, Oxford, having completed her PhD in Theoretical Atomic and Nuclear Physics in 1994.
